Minion of Mayhem is an account-wide title that was added in the World of Warcraft expansion and is obtainable through pet battles. There are 2 ways to get this title:
Minion of Mayhem used to be unique and extremely hard to get title, but with the release of Shadowlands expansion, it was made easier.

To obtain Minion of the Mayhem title in the old way players need to complete 5 scenarios: Call from the Depths, The Dead Grotto, Gnomeregan, Stratholme, and Blackrock Depths (in that order). A significant collection of pets and knowledge of tactics will be required to complete them. When players reach the final stage in Blackrock Depths, before exiting the dungeon, talk to Tasha Riley and select the dialog option “Join COUNCIL OF CHAOS!”
Crafting Capal pet requires leveling reputation with Maldraxxus faction in order to purchase the ingredients required for crafting. You’ll also need Animated Ulna, a reward for Maldraxxus rare world quest. Finally, loot Flexing Phalanges item. Right-click on any of them to craft your Capal pet.
